Refer a patient

To refer a patient to the Children’s Hospital, contact our MD Network at 800-233-4082. We are available 24 hours a day, seven days a week and can assist with appointments, admissions, transfers, and consultations.

Physician Liaison Program

Physician liaisons bridge the gap between referring physicians and the hospital and serve as a single point of contact for referring physicians to communicate with.

When requesting an inpatient transfer, please anticipate the following from the Transfer Center:

  • Information gathering: patient name, date of birth, referring hospital name, referring physician name, contact information, insurance information and reason for referral
  • Connection to the appropriate attending physician in specified service line
  • Bed reservation for accepted patients
  • Assistance to arrange transportation
  • Patient transfer checklist (send the following along with the patient):
    • History and physical
    • Progress notes
    • Consult notes
    • Medication list
  • Results from all studies, imaging, and labs performed during care
  • For registration purposes, please fax the patient’s face sheet to Penn State Health Milton S. Hershey Medical Center Transfer Center at 717-531-0125.

Real-time access to patient information

My Patients is a web-based application for referring physicians that provides real-time access to patient information from inpatient or outpatient visits at Penn State Health Children’s Hospital.

How it works

When a patient is registered for an inpatient or outpatient visit, he or she is asked to identify their primary care or referring doctor at that time. The web-application acknowledges the referring physician’s connection to the patient and gives the doctor access to view the patient’s data.
