The Penn State Hershey Diabetes Playbook

Penn State Hershey Diabetes Institute is focused on Reinventing Diabetes Care™. One of the ways we are reaching this goal is by hearing what our patients have to say. When our patients asked for an easy way to keep track of their diabetes care the Diabetes Playbook was made. Not only does the book offer tips on how to manage diabetes from people who treat diabetes, it offers tips from people who live with diabetes every day. The Diabetes Playbook is a Gold winner in the 24th Annual Healthcare Marketing Advertising Report Awards, recognized for its effective way of delivering its diabetes education message.

Medicine in Blue and White

"Medicine in Blue and White" is a television series about Penn State Milton S. Hershey Medical Center that airs on the Big Ten Network.  The the first of three episodes, personal stories from children, parents, staff, and medical professionals at Penn State Hershey share their experiences with the world.

In this episode, Dr. Heather Stuckey explains the importance of utilizing social media as a way to help control one's Diabetes.  Online forums, along with the Penn State Hershey Diabetes Playbook provides education to the patient as well as multiple resources.

The Patient Centered Medical Home

The Patient Centered Medical Home (PCMH) is a new model of patient care. According to Joint Principles adopted nationally in 2007, hallmarks of the Medical Home include:

  • Continuous and comprehensive care directed by one’s primary care physician in partnership with patients and their families.
  • Care that is coordinated across all types of care and across all stages of life.
  • Care that is available when you need it.
  • Care that follows evidence-based medicine and is committed to continuous quality improvement.
Many primary care practices in Pennsylvania have been nationally recognized as PCMHs, and many have participated in initiatives to learn about becoming Medical Homes. The largest of these initiatives is the Pennsylvania Chronic Care Initiative led by state government and involving more than 150 practices statewide.
 
Because Pennsylvania has been a leader in advancing the PCMH, Pennsylvania was selected as one of four states to develop a national model for a Primary Care Extension Service to support primary care practices in becoming Medical Homes.
 
This work is being led at the Penn State College of Medicine by Robert A. Gabbay, MD, PhD, Director of the Penn State Hershey Diabetes Institute. Dr. Gabbay has been faculty chair of the PA Chronic Care Initiative, is a member of the core project team, and was a leader in Pennsylvania’s Chronic Care Management, Reimbursement and Cost Reduction Commission.
 
Dr. Gabbay has a grant to evaluate the impact of the PA Chronic Care Initiative in the first regional rollout in Southeastern PA. He is also helping Penn State Hershey Department of Family and Community Medicine train medical students in the PCMH.
