An interested patient should obtain a referral from his or her physician's office and an authorization from the insurance company to ensure insurance coverage. Our program is recognized by the American Diabetes Association. The following education opportunities are offered through our program.
One-on-One Session with Diabetes Nurse Educator
Individual one-hour session with diabetes nurse educator –review of how to treat high or low blood sugar, sick-day guidelines, etc.
New to Type 2 Diabetes
Two-hour class taught by diabetes nurse educator – review of basic physiology of diabetes, management tools to include meal plan guidelines, physical activity impact, medications, and glucose meters
Meter Class
One-hour session taught by diabetes nurse educator to learn how to use a glucometer to check your blood sugars
Insulin/Byetta Initiation Class
One-hour session taught by diabetes nurse educator to learn how to give yourself insulin or Byetta injections
